This Online Form is designed so that you can enter your Personal Information and
have it sent directly to the ALUMNI Club..
This form is separate from the CLASSMATES.COM web site and will be used for two
purposes. First, to record you as a Classmate and
second, to record your interest in receiving Information and/or applying for
Alumni Club member status.
All information will be kept confidential. Mailing, including e-mail, if you
prefer, will be sent to you over the next year about the Activities of the
Class and Events which may be planned. For those
who are planning to attend an Alumni Event, further
information will be requested..
This will come in the form of a paper form, so your Mailing Address is required
for this to occur. Wee cannot admit anyone to membership without
a legal mailing address, supplied to the membership committee.
Any and all information requested should be filled in below. If you prefer NOT
to submit some personal information, you may leave those fields
